Case Study A – Worker killed by faulty forklift truck

A worker was killed on his first day at work when the forklift truck he was driving overturned and crushed him.

The forklift truck was unsuitable for use on uneven ground or loose surfaces and further there was no company policy that operators must wear seatbelts.  Such a tragedy could have been prevented if a seatbelt policy had been in operation and suitable vehicles used for the purpose.  The Company was prosecuted by the Health and Safety Executive.
