Case Study A – Ladder Incorrectly Used 

A worker was killed when he fell from a ladder, which had been incorrectly fitted, during an inspection of a boiler in the loft of a house.    The omission of a retaining bar on the ladder (which is a safety device that locks the ladder) caused the ladder to slide forwards and the worker to fall backwards, sustaining fatal head injuries. 

Case Study B – Ladder unsuitable equipment for the job 

A worker suffered a fractured skull resulting in brain damage when he fell from a ladder while replacing roof tiles on a two story block of flats.  The employer was prosecuted under Health and Safety legislation.   

In jobs of a short duration where there is a low risk ladders can be an option to consider, but in this particular situation they were not suitable – a mobile scaffolding tower or elevated working platform should have been used.  

Case Study C – Unsecured Ladder 

A worker, descending an unsecured ladder, was forced to jump when it slipped from under him.  

The worker sustained a serious fracture to his foot resulting in him being off work sick for a lengthy period of time.     

The Company was in breach of the Work at Height Regulations 2005 by not undertaking a specific risk assessment and for failing to put in place a procedure to ensure that the work was properly planned.
